9/16/2011 - AFS PHI JENESYS VS 2011 - Required Documentation (for Students))
Dear JENESYS VS 2011 Student Finalists & Alternates,
Please see below list of required documentation. Kindly submit on or before September 30, 2011.
-original Philippine passport valid up to 6 months beyond the date of program end (June 18, 2012 and beyond)
-2 sets of original 2011 NSO birth certificates; or proof late registration of birth, as may apply
-original 2011 NSO marriage certificate, or original notarized affidavit of Single Parenthood, or proof of legal guardianship, as may apply
-clear copy of baptismal certificate
-original 2011 high school transcript of records / Form 137 or 138
-original 2011 DSWD Clearance (Travel Alone for Minor)
-2 sets original Parental Travel Permit, signed by both parents/legal guardian, as may apply
-complete record of immunizations, including seasonal flu AND AH1N1 vaccinations
-TB (skin or blood test) results (and additional chest X-ray results, if and wjen TB test is positive)
-2 pcs Japan visa-sized photos (do-it-yourself printing will not be acceptable)
Note on Passport Applications: If one parent and the student applies for passports at the same time, this will lessen the paperwork needed to be presented to the DFA. Please apply for EXPEDITE processing of passport. Reminder on expired passport: please submit new details of renewed passports.
Note on DSWD Clearance: Please apply for Unaccompanied Minor or Minor Traveling Alone clearance. The reason for this is because if AFS provides chaperon teacher passport to process your DSWD clearance and then teacher for some reason at the last minute is unable to travel, it will result to all the 56 students in the group will be unable to travel because their clearances would be tied up to the name of the said chaperon teacher.
Compliance: It is important that finalists/alternates complete name is reflected ‘as in passport’ in all documents. There will be more documents required by AFS Japan and MOFA in the coming days but this list above will give finalists/alternates a headstart in compliance as announced previously in the selection camps.
Endorsement and Permission School: Compliant finalists/alternates should have written permission from their school to attend the local AFS PHI events from the Dec 4-6 (pre-dep orientation) as well as the Dec 18-20 (re-entry orientation), in Manila, in addition to the December 6-18 cultural exchange in Japan.
